Acute chest pain (ACP) is a common symptom encountered in clinical practice, often challenging physicians due to its diverse etiology. This article aims to provide a comprehensive approach to the evaluation and management of ACP, emphasizing the importance of a systematic, patient-centered approach.
The initial evaluation of a patient with ACP should focus on identifying life-threatening conditions. A thorough history and physical examination, coupled with an electrocardiogram (ECG) and biomarker assessment, can help discern high-risk conditions such as acute coronary syndromes (ACS), pulmonary embolism, and aortic dissection.
Further diagnostic testing should be guided by the initial evaluation. For patients with a high pretest probability of ACS, an early invasive strategy is recommended. Alternatively, patients with a low to intermediate risk may benefit from non-invasive testing such as stress testing or coronary computed tomography angiography (CCTA).
Risk stratification using validated tools like the HEART score can guide disposition decisions. Patients with a low-risk HEART score can be safely discharged with close outpatient follow-up, while those with intermediate or high-risk scores warrant hospitalization and further evaluation.
The management of ACP should be tailored to the underlying cause. For patients diagnosed with ACS, management should focus on revascularization and secondary prevention measures. In contrast, non-cardiac chest pain may be managed with a multidisciplinary approach involving pain management, psychiatry, and gastroenterology services.
In conclusion, a comprehensive and systematic approach to ACP can facilitate accurate diagnosis, appropriate risk stratification, and effective management. Emphasizing a patient-centered approach that incorporates shared decision-making can improve patient outcomes and satisfaction.
