Anemia, a condition characterized by a decrease in the number of red blood cells or less than the normal quantity of hemoglobin in the blood, is a common clinical problem encountered in everyday practice. Its etiology is multifactorial, and its diagnosis requires a comprehensive approach. This article aims to provide a systematic guide to the diagnostic approach for anemia in clinical practice.
Anemia affects approximately one-third of the global population, with the highest prevalence in preschool-aged children and women. It can result from three broad mechanisms: decreased red cell production, increased red cell destruction, or blood loss. In clinical practice, it is often a combination of these factors, complicating the diagnostic process.
The clinical presentation of anemia is varied and often nonspecific, making it a diagnostic challenge. Symptoms can range from fatigue, pallor, and dyspnea to more severe manifestations such as congestive heart failure in severe cases. The physical examination may reveal signs of specific causes, such as petechiae in aplastic anemia or jaundice in hemolytic anemia.
The initial approach to diagnosing anemia involves a complete blood count (CBC) and reticulocyte count. The CBC provides information on the degree of anemia, red cell indices, and the presence of any leukocyte or platelet abnormalities. The reticulocyte count helps determine whether the bone marrow response is adequate.
Further diagnostic evaluation is guided by the red cell indices. Microcytic anemia is often due to iron deficiency or thalassemia, while macrocytic anemia can be caused by vitamin B12 or folate deficiency, myelodysplastic syndrome, or certain medications. Normocytic anemia requires a more detailed evaluation, including assessment of kidney function and iron studies.
A bone marrow examination is not routinely required in the evaluation of anemia. It is reserved for cases where the diagnosis remains unclear after initial investigations, or when there is suspicion of a primary bone marrow disorder.
Anemia is a complex clinical problem with a myriad of potential causes. A systematic approach to its diagnosis, beginning with a thorough history and physical examination, followed by targeted laboratory investigation, is key to identifying the underlying etiology. Understanding the epidemiology, clinical presentation, and diagnostic approach to anemia is essential for effective patient management in clinical practice.
