Cardiovascular disease (CVD) represents a significant health burden for patients with diabetes. Effective risk assessment and mitigation strategies are crucial for improving patient outcomes. This review provides an overview of current approaches and emerging trends in this field.
Cardiovascular risk assessment in diabetic patients involves the evaluation of traditional risk factors such as age, sex, hypertension, dyslipidemia, and smoking. But importantly, it also includes diabetes-specific factors like duration of diabetes, presence of end-organ damage, and glycemic control. Tools like the UKPDS risk engine and the ACC/AHA ASCVD risk calculator are commonly used to quantify cardiovascular risk.
Effective risk mitigation in diabetic patients begins with optimal glycemic control, achieved through lifestyle modifications and pharmacological interventions. Hypertension and dyslipidemia should be aggressively managed, with targets often being stricter than in the general population. Antiplatelet therapy is recommended for secondary prevention and in certain high-risk primary prevention scenarios.
New classes of glucose-lowering drugs, such as SGLT2 inhibitors and GLP-1 receptor agonists, have shown promising cardiovascular benefits. Their use is now recommended in patients with type 2 diabetes and established CVD or high cardiovascular risk. Furthermore, the concept of 'diabetes remission' through intensive lifestyle intervention or metabolic surgery is gaining traction, offering potential for significant cardiovascular risk reduction.
Cardiovascular risk assessment and mitigation is a cornerstone of diabetes management. While traditional risk factors remain important, the role of diabetes-specific factors and novel therapeutic agents is increasingly recognized. Future research should focus on refining risk assessment tools and exploring innovative risk reduction strategies, with the ultimate aim of improving cardiovascular outcomes in this high-risk population.
