Abstract

Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disease that significantly impacts patient quality of life. Recent developments in understanding its immunopathogenesis have led to advances in diagnostic approaches and the emergence of targeted therapies. This review discusses the current epidemiology, pathophysiology, risk factors, clinical features, and diagnostic criteria of AD, with an emphasis on recent advances in management, including novel biologic and small molecule therapies. Evidence-based guideline recommendations and practical clinical insights are highlighted to provide a comprehensive update for healthcare professionals managing this complex dermatological condition.

Introduction

Atopic dermatitis is a prevalent, multifactorial skin disorder characterized by pruritus, eczematous lesions, and a chronic-relapsing course. Affecting both children and adults, AD has a profound psychosocial and economic burden. Over the past decade, research has elucidated critical mechanisms underlying the disease, resulting in improved diagnostic tools and transformative therapies. This review synthesizes recent evidence and guideline-based approaches to provide clinicians with updated insights into the diagnosis and management of AD.

Epidemiology / Disease Burden

AD affects approximately 15–20% of children and 1–10% of adults globally, with increasing prevalence in industrialized nations. The disease commonly manifests in early childhood, with up to 60% of cases developing within the first year of life. While some patients outgrow AD, a significant proportion experience persistent or relapsing disease into adulthood. The associated burden extends beyond cutaneous symptoms, contributing to sleep disturbances, psychological distress, and increased risk for comorbidities such as asthma and allergic rhinitis. Healthcare utilization and productivity losses underscore the need for effective management strategies.

Pathophysiology

The pathogenesis of AD involves complex interactions between genetic predisposition, barrier dysfunction, immune dysregulation, and environmental triggers. Filaggrin gene (FLG) mutations compromise epidermal barrier integrity, increasing transepidermal water loss and susceptibility to allergens and microbes. Immune dysregulation is marked by type 2 helper T cell (Th2)-mediated inflammation, leading to elevated cytokines such as interleukin (IL)-4, IL-13, and IL-31. Chronicity is driven by shifts toward Th1 and Th22 responses, perpetuating skin barrier impairment and inflammation. The role of the skin microbiome, particularly Staphylococcus aureus colonization, further exacerbates disease severity.

Risk Factors

Major risk factors for AD include a personal or family history of atopy, FLG mutations, urban living, and exposure to environmental allergens or pollutants. Early-life factors such as cesarean delivery, antibiotic exposure, and lack of breastfeeding may increase susceptibility. Psychosocial stress and climatic factors can precipitate disease flares. Recognizing these risk factors assists clinicians in identifying at-risk populations and tailoring preventive strategies.

Clinical Features

AD is typified by pruritic, erythematous, and scaly lesions with a predilection for flexural areas in older children and adults, and facial/extensor involvement in infants. Chronic scratching leads to lichenification, excoriations, and secondary infections. The clinical presentation is heterogeneous, ranging from mild intermittent eczema to severe, widespread disease. Associated features include xerosis, Dennie–Morgan folds, and hyperlinear palms. Disease activity and severity can be assessed using validated tools such as the Eczema Area and Severity Index (EASI) and Scoring Atopic Dermatitis (SCORAD).

Diagnosis

Diagnosis of AD is clinical, based on characteristic morphology, distribution, and chronic-relapsing course. Widely accepted criteria, such as those by Hanifin and Rajka, emphasize pruritus, typical lesions, chronicity, and atopic history. Differential diagnosis includes contact dermatitis, seborrheic dermatitis, psoriasis, and cutaneous T-cell lymphoma. Laboratory investigations are generally not required, but elevated serum IgE and eosinophilia may support the diagnosis in atypical cases. Patch testing and skin biopsies may be considered to exclude alternative etiologies.

Treatment & Management

Management of AD is multimodal, encompassing patient education, trigger avoidance, skin barrier restoration, anti-inflammatory therapy, and management of secondary infection. Regular use of emollients to restore barrier function is fundamental. Topical corticosteroids remain first-line anti-inflammatory agents, with topical calcineurin inhibitors as steroid-sparing options. For moderate-to-severe disease, phototherapy and systemic immunosuppressants (e.g., cyclosporine, methotrexate) may be indicated. Addressing pruritus, sleep disturbance, and psychosocial stress is essential for holistic care. Antimicrobial therapy is reserved for overt secondary infections.

Recent Advances / Emerging Therapies

In recent years, understanding of AD immunopathogenesis has facilitated the development of targeted therapies. Biologic agents such as dupilumab, an IL-4 receptor alpha antagonist, have demonstrated significant efficacy and safety in moderate-to-severe AD, leading to durable improvements in skin clearance and quality of life. Other emerging biologics targeting IL-13 (tralokinumab, lebrikizumab) and IL-31 receptor (nemolizumab) are under investigation. Small molecule Janus kinase (JAK) inhibitors (e.g., upadacitinib, abrocitinib) offer oral alternatives with rapid onset of action, though long-term safety data are evolving. These advances are reshaping the therapeutic landscape and providing hope for patients with refractory disease.

Guideline Recommendations

International guidelines from the American Academy of Dermatology (AAD), European Dermatology Forum (EDF), and others emphasize a stepwise approach to AD management, prioritizing skin care, avoidance of triggers, and escalation to topical or systemic therapies based on severity. Dupilumab and other biologics are recommended for patients with inadequate response to conventional therapies. Patient education, proactive maintenance, and multidisciplinary care are integral to improving outcomes. Regular monitoring for adverse effects and comorbidities is advised, particularly with systemic immunosuppression or newer agents.

Conclusion

Atopic dermatitis remains a challenging, multifaceted disease with substantial individual and societal impact. Advances in mechanistic understanding and innovative therapeutics are transforming the standard of care, enabling more precise and effective management. Ongoing research into biomarkers, personalized medicine, and long-term safety will further refine strategies for optimal patient outcomes. Clinicians must remain abreast of evolving guidelines and emerging therapies to deliver evidence-based, patient-centered care for individuals with AD.
