Additionally, guideline recommendations and practical clinical implications are analyzed, offering a comprehensive resource for healthcare professionals seeking to optimize patient outcomes through family-centered care.
Chronic diseases such as diabetes, cardiovascular disease, and chronic obstructive pulmonary disease (COPD) are leading causes of morbidity and mortality worldwide. Traditional chronic care models often focus primarily on the individual, with limited attention to the broader social and familial context. In recent years, there has been growing recognition of the pivotal role family dynamics play in disease adaptation and management. Family resilience-based chronic care models aim to leverage family strengths, communication, and problem-solving skills to improve the overall quality of life (QoL) for patients and their support systems. This article explores the scientific rationale, clinical relevance, and empirical support for integrating family resilience into chronic care frameworks.
The global burden of chronic diseases continues to rise, with an estimated 71% of all deaths worldwide attributed to non-communicable diseases (NCDs), according to the World Health Organization (WHO).
