Acute chest pain (ACP) is a common yet challenging presentation in emergency departments. Its etiology ranges from benign conditions to life-threatening diseases, making a prompt and precise diagnosis crucial. This article elucidates a comprehensive approach to diagnosing and managing ACP.
Firstly, a thorough history and physical examination are essential. The nature, onset, location, and radiation of the pain, associated symptoms, and risk factors for cardiovascular disease should be elicited. Physical examination should focus on vital signs, heart and lung examination, and signs of peripheral vascular disease.
Electrocardiogram and cardiac biomarkers are the first-line diagnostic tests. They should be performed immediately upon suspicion of ACP. Other investigations like chest radiography, computed tomography, and echocardiography should be considered based on clinical suspicion.
Risk stratification is crucial to guide management. Various validated clinical decision rules such as the HEART score, TIMI risk score, etc., can be utilized. These tools combine clinical variables and diagnostic test results to predict the likelihood of major adverse cardiac events.
Management depends on the underlying cause. In cases of acute coronary syndrome, immediate revascularization is often required. Non-cardiac causes should be managed accordingly. Regardless of the cause, all patients should receive symptomatic treatment, risk factor modification, and appropriate follow-up.
ACP is a complex clinical entity requiring a systematic approach for optimal patient outcomes. A thorough history, physical examination, appropriate diagnostic testing, and risk stratification form the cornerstone of its management. As physicians, we must remain vigilant and adaptable in our approach to this common yet challenging clinical presentation.
