Chronic cough, defined as a cough lasting more than eight weeks, is a common yet challenging condition to manage in clinical practice. It often leads to significant morbidity, affecting patients' quality of life and causing considerable diagnostic and therapeutic dilemmas for clinicians.
Chronic cough is typically associated with three primary etiologies: asthma, gastroesophageal reflux disease (GERD), and upper airway cough syndrome (UACS). These conditions often coexist, complicating diagnosis and management. The pathophysiology of chronic cough involves a complex interplay between the cough reflex pathway and various triggers, leading to hypersensitivity and hyperresponsiveness of the cough reflex.
An effective diagnostic approach begins with a thorough clinical history and physical examination. This is followed by initial testing to rule out common causes, such as chest radiography for suspected lung disease and spirometry for asthma. If these tests are inconclusive, further investigations may include high-resolution computed tomography, bronchoscopy, or esophageal pH monitoring for GERD. Empirical treatment trials can also provide diagnostic clues.
Management of chronic cough focuses on treating the underlying cause. For asthma, inhaled corticosteroids are first-line therapy. Proton pump inhibitors are used for GERD, and antihistamines or nasal steroids for UACS. In refractory cases, neuromodulators like gabapentin or amitriptyline can be considered to reduce cough reflex sensitivity. Patient education and reassurance are vital components of management.
Chronic cough is a complex condition requiring a comprehensive and systematic approach for effective diagnosis and management. Understanding the common etiologies, pathophysiology, and employing a stepwise diagnostic approach can aid in identifying the underlying cause. Tailoring the treatment to the identified cause, along with patient education, can significantly improve outcomes and patient satisfaction.
