The article further explores recent advances in therapeutic interventions and provides practical guideline-based recommendations for healthcare professionals managing these challenging clinical scenarios.
Substance-related critical events, such as drug overdoses, intoxications, and withdrawal syndromes, are increasingly prevalent causes of acute medical and neuropsychiatric morbidity. These events can precipitate profound neurobehavioral sequelae, ranging from cognitive and executive dysfunction to affective and psychomotor disturbances. Understanding the pathways and determinants of neurobehavioral recovery is critical for optimizing outcomes and guiding multidisciplinary care in affected individuals. This review aims to provide clinicians and researchers with a comprehensive overview of the epidemiology, pathophysiology, clinical features, and therapeutic options relevant to neurobehavioral recovery after substance-related critical events.
Substance-related critical events represent a major public health concern, with the World Health Organization estimating over 35 million people globally affected by substance use disorders (SUDs). Acute substance intoxications, overdoses, and withdrawal syndromes account for significant emergency department visits and inpatient admissions. The opioid epidemic, benzodiazepine misuse, and rising stimulant-related emergencies have further increased the burden, with associated neurobehavioral impairments contributing to substantial disability, healthcare utilization, and socioeconomic costs. Notably, neurobehavioral sequelae often persist long after acute stabilization, underscoring the need for targeted recovery pathways and long-term monitoring.

Standardized neuropsychological batteries (e.g., MoCA, MMSE, or more comprehensive tools) are essential for quantifying cognitive domains. Neuroimaging (MRI, CT, PET) may detect structural or metabolic brain changes, and EEG can help exclude subclinical seizures or non-convulsive status epilepticus. Biomarkers such as S100B or neuron-specific enolase are being explored for prognostication. Management strategies are individualized and multidisciplinary, targeting both the underlying substance use disorder and neurobehavioral deficits. Longitudinal follow-up ensures monitoring for relapse, psychiatric comorbidities, and social reintegration challenges.
Recent advances have focused on neuroprotective agents (e.g., amantadine, memantine), anti-inflammatory strategies, and modulators of neurotrophic factors to enhance recovery. Digital therapeutics, tele-rehabilitation, and app-based cognitive training platforms have shown promise in augmenting traditional interventions.
