This complexity increases the risk of care fragmentation—defined as disjointed, poorly coordinated services that compromise quality, safety, and patient satisfaction. Case-based learning (CBL) has emerged as a powerful educational modality to train healthcare professionals in recognizing and preventing care fragmentation. By anchoring learning within real-world scenarios, CBL promotes critical thinking, contextual application of guidelines, and interprofessional teamwork. CBL has evolved with virtual simulation tools, enabling multidisciplinary teams to train remotely on complex cases. Emerging therapies focus on social determinants of health, embedding social workers and community health advocates within care teams. New payment models, such as bundled payments and accountable care organizations, incentivize coordinated care and reduce fragmentation. Ongoing research explores the impact of artificial intelligence on predicting and preventing fragmentation through real-time analytics.
Leading professional bodies, including the Institute for Healthcare Improvement and the Agency for Healthcare Research and Quality, recommend structured team-based care, standardized communication protocols, and active patient engagement. The adoption of CBL is endorsed as a means to enhance interprofessional education and improve system-level integration.
