Type 2 diabetes mellitus (T2DM) represents a growing global health challenge, with high-risk populations disproportionately affected. Effective prevention strategies are crucial for mitigating disease burden, morbidity, and associated healthcare costs. This review synthesizes current epidemiological data, pathophysiological mechanisms, established risk factors, clinical features, diagnostic modalities, and evidence-based management approaches. Emphasis is placed on recent advances, emerging therapies, and contemporary guideline recommendations tailored to high-risk groups. The goal is to provide clinicians with a comprehensive framework for early identification, risk stratification, and implementation of preventive interventions in those most susceptible to developing T2DM.
Type 2 diabetes mellitus is a multifactorial metabolic disorder characterized by chronic hyperglycemia, resulting from insulin resistance and progressive beta-cell dysfunction. Its prevalence is rising alongside urbanization, aging populations, and lifestyle changes, creating an urgent need for effective prevention particularly among high-risk individuals. Early identification and targeted interventions can substantially reduce the progression to overt diabetes and its associated complications.
Globally, over 537 million adults are living with diabetes, of which more than 90% have T2DM. The International Diabetes Federation projects this number will rise to 643 million by 2030 if current trends persist. High-risk populations including individuals with obesity, impaired glucose tolerance, family history of diabetes, specific ethnic backgrounds (e.g., South Asians, African Americans, Hispanic/Latino), and women with a history of gestational diabetes face a markedly elevated incidence. The public health impact is profound, with T2DM contributing significantly to cardiovascular disease, renal failure, blindness, and lower-extremity amputations, as well as to escalating healthcare expenditures worldwide.
The pathogenesis of T2DM in high-risk groups is complex, involving a dynamic interplay between genetic predisposition and environmental factors. Central to disease development is insulin resistance in skeletal muscle, liver, and adipose tissue, accompanied by relative or absolute pancreatic beta-cell dysfunction. Chronic low-grade inflammation, oxidative stress, dysregulation of adipokines (such as adiponectin and leptin), and alterations in gut microbiota further modulate disease risk. In high-risk populations, these mechanisms are often amplified, leading to earlier and more severe metabolic derangements.
Key risk factors for T2DM include advancing age, overweight and obesity (particularly central adiposity), sedentary lifestyle, unhealthy diet, family history of diabetes, and certain ethnicities. Additional risk factors in high-risk groups encompass history of gestational diabetes, polycystic ovary syndrome, hypertension, dyslipidemia, and the use of diabetogenic medications such as glucocorticoids or atypical antipsychotics. Socioeconomic status and psychosocial stressors also contribute, underscoring the multifaceted nature of diabetes risk.
In the prediabetic phase, individuals are frequently asymptomatic, although subtle manifestations such as fatigue, mild polyuria, or blurred vision may occur. Clinical recognition is often delayed until complications arise. In high-risk populations, the presence of acanthosis nigricans (indicative of insulin resistance), central obesity, or features of metabolic syndrome should heighten clinical suspicion. Early identification in this group is paramount, as prolonged undiagnosed hyperglycemia increases the risk of microvascular and macrovascular sequelae.
Diagnosis of prediabetes and diabetes relies on standardized criteria: fasting plasma glucose (FPG), 2-hour plasma glucose during a 75-g oral glucose tolerance test (OGTT), and glycated hemoglobin (HbA1c). For high-risk populations, the American Diabetes Association (ADA) recommends screening adults aged ≥35 years, or earlier in those with additional risk factors. The threshold for intervention is lower in these groups, reflecting their heightened susceptibility. Point-of-care HbA1c testing and risk calculators can facilitate opportunistic screening in primary care and community settings.
The cornerstone of diabetes prevention in high-risk populations is intensive lifestyle modification, including dietary changes (reduced caloric intake, increased fiber, limited sugar-sweetened beverages), regular physical activity (at least 150 minutes per week of moderate-intensity exercise), and weight loss (5–10% of body weight). Pharmacological interventions, notably metformin, are recommended for select high-risk individuals, particularly those under 60 years of age with a BMI ≥35 kg/m² or women with prior gestational diabetes. Structured education, behavioral counseling, and culturally tailored interventions enhance adherence and efficacy.
Recent advances include novel pharmacotherapies (e.g., GLP-1 receptor agonists, SGLT2 inhibitors) demonstrating glycemic benefits, weight reduction, and cardiovascular protection in prediabetic and high-risk cohorts. Digital health platforms, wearable technologies, and mobile applications provide scalable tools for lifestyle coaching, self-monitoring, and real-time feedback. Community-based interventions and the use of population health data for risk stratification are gaining momentum, allowing for targeted prevention strategies in diverse settings. Ongoing research into gut microbiome modulation, anti-inflammatory agents, and genetic risk profiling holds promise for future individualized prevention approaches.
Leading organizations, including the ADA, World Health Organization (WHO), and National Institute for Health and Care Excellence (NICE), advocate for early identification of at-risk individuals using validated screening tools. Lifestyle intervention remains first-line, with metformin considered in those with persistent hyperglycemia despite non-pharmacologic measures. Individualization of care, periodic reassessment of risk, and integration of multidisciplinary support are emphasized. Emerging guidelines support the incorporation of digital and community-based resources, especially in underserved or high-risk populations.
Preventing T2DM in high-risk populations requires a multifaceted, evidence-based approach combining lifestyle modification, pharmacotherapy, and systems-level interventions. Early risk identification, personalized strategies, and adherence to evolving guidelines are essential for effective prevention and long-term risk reduction. Ongoing research and innovation will further refine prevention paradigms, offering renewed hope for curbing the global diabetes epidemic among those most vulnerable.
