Abstract

Healthy longevity is increasingly recognized as a primary goal of geriatric medicine, transcending mere lifespan extension to promote quality of life and functional independence among older adults. Integrated geriatric care, characterized by multidisciplinary collaboration and individualized management, has emerged as a cornerstone in optimizing aging trajectories. This review synthesizes current evidence on the impact of integrated care models on geriatric outcomes, examining epidemiology, pathophysiology, risk stratification, clinical manifestations, diagnostic criteria, therapeutic strategies, and guideline recommendations. Emphasis is placed on mechanisms underpinning age-related decline and the clinical implications of recent advances in the field. Practical insights are provided to inform effective, patient-centered care for the aging population.

Introduction

The demographic shift towards an aging population presents significant challenges and opportunities for healthcare systems worldwide. By 2050, individuals aged 65 years and older are projected to constitute over 16% of the global population, necessitating a paradigm shift in care delivery. Healthy longevity denotes not only increased lifespan but also enhancement of healthspan years lived free from major disease and disability. Integrated geriatric care, involving coordinated input from medical, nursing, allied health, and social services, is foundational to achieving this goal. This article provides an in-depth review of the scientific and clinical underpinnings of healthy longevity trajectories facilitated by integrated care.

Epidemiology / Disease Burden

Population aging is associated with a marked increase in chronic diseases, frailty, and multimorbidity. Diagnosis in geriatric care extends beyond disease labeling to encompass multidimensional assessment of medical, functional, cognitive, emotional, and social domains. Tools such as the Frailty Index, Mini-Mental State Examination (MMSE), and Geriatric Depression Scale (GDS) aid in systematic evaluation. Laboratory and imaging studies are tailored to minimize harm and maximize diagnostic yield, with emphasis on shared decision-making and consideration of patient goals and preferences.

Treatment & Management

Integrated geriatric care prioritizes individualized, goal-directed interventions. Core components include medication optimization (deprescribing where appropriate), physical rehabilitation, nutritional support, chronic disease management, cognitive stimulation, and psychosocial support. Interdisciplinary team meetings and care coordination are critical for addressing complex needs and preventing care fragmentation. Advance care planning and palliative care integration ensure alignment with patient values across the continuum of care. Evidence supports that integrated care models reduce hospitalizations, improve functional outcomes, and enhance patient and caregiver satisfaction.

Recent Advances / Emerging Therapies

Recent advances in geriatric medicine include the use of digital health technologies, remote monitoring, and artificial intelligence to enable proactive risk stratification and personalized intervention. Pharmacological innovation targets aging pathways, with agents such as senolytics and metformin under investigation for their potential to modulate biological aging. Multimodal interventions combining physical activity, cognitive training, and nutritional strategies are showing promise in delaying frailty and cognitive decline. Implementation of comprehensive geriatric assessment in acute care settings has demonstrated reductions in mortality and institutionalization rates.

Guideline Recommendations

International and national guidelines advocate for routine use of comprehensive geriatric assessment, individualized care planning, and multidisciplinary management for older adults with complex needs. The American Geriatrics Society and European Geriatric Medicine Society emphasize the importance of frailty screening, medication review, falls prevention, and integrated palliative care. Clinical pathways should be adapted to local resources and patient preferences, with continuous quality improvement and outcome monitoring.
