Chronic cough, defined as a cough lasting over eight weeks, is a common and challenging condition that often frustrates both patients and clinicians. Despite its prevalence, the complex pathophysiological mechanisms behind chronic cough remain elusive, complicating diagnosis and treatment. This article aims to outline a comprehensive approach to diagnosing and treating chronic cough in clinical practice.
A detailed patient history and physical examination are the cornerstones of diagnosing chronic cough. Identifying potential triggers, such as allergens, gastroesophageal reflux disease (GERD), and medications (notably ACE inhibitors) can provide crucial diagnostic clues. Objective tests, such as chest X-rays, spirometry, and bronchoprovocation tests, may also be informative. In some cases, more specialized investigations, such as high-resolution computed tomography (HRCT), bronchoscopy, or esophageal pH monitoring, may be required.
Treatment of chronic cough should be tailored to the underlying cause. For coughs associated with GERD, proton pump inhibitors may be beneficial. In patients with cough variant asthma or eosinophilic bronchitis, inhaled corticosteroids may be effective. Antihistamines and decongestants can be used in patients with postnasal drip syndrome. Chronic cough refractory to these measures may require trials of neuromodulatory agents, such as gabapentin or amitriptyline.
Despite these strategies, chronic cough remains a therapeutic challenge. Many patients remain symptomatic despite extensive evaluation and treatment trials, highlighting the need for new therapeutic strategies. Recent advances in our understanding of the neurophysiology of cough have identified novel potential targets, including the transient receptor potential (TRP) channels and the P2X3 receptor, opening new avenues for future research and drug development.
Chronic cough is a complex condition requiring a comprehensive, patient-centered approach to diagnosis and treatment. While current strategies can be effective, many patients remain symptomatic, underscoring the need for continued research into the pathophysiology of chronic cough and the development of novel therapeutic strategies.
