Acute chest pain (ACP) is a common clinical presentation that poses a significant diagnostic challenge for clinicians. Its broad differential diagnosis, ranging from benign musculoskeletal conditions to life-threatening emergencies like acute coronary syndrome (ACS), necessitates a comprehensive and systematic approach.
Understanding the patient's history is paramount in the evaluation of ACP. Aspects such as the quality, location, duration, and radiation of pain, coupled with aggravating and relieving factors, can provide valuable clues towards the underlying etiology. The patient's medical history, including risk factors for cardiovascular disease, should also be considered.
Physical examination is a crucial component in the evaluation of ACP. Findings such as a new murmur, unequal pulses, or signs of heart failure may suggest a specific diagnosis. However, a normal examination does not exclude serious causes of chest pain.
Electrocardiogram (ECG) and cardiac biomarkers are the primary diagnostic tools for suspected ACS. Other investigations like chest X-ray, computed tomography (CT) scan, or echocardiography may be required based on the clinical suspicion of other potential causes.
Management of ACP is largely determined by the underlying cause. In suspected ACS, prompt revascularization is the cornerstone of treatment. Other conditions may require medical management, surgical intervention, or simply reassurance and follow-up.
ACP is a complex clinical presentation requiring a thorough and systematic approach. A detailed history, physical examination, and appropriate use of diagnostic tools are key to identifying the underlying cause. Timely and appropriate management can significantly improve patient outcomes. Continued research and education are essential to refine our approach to this common clinical conundrum.
