Introduction

Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, remains a significant health burden, particularly in developing countries. Despite advancements in diagnostic methods and treatment protocols, challenges persist. This article aims to provide an updated approach to its diagnosis and management.

Diagnostic Methods

While blood culture remains the gold standard for diagnosis, its sensitivity is variable, ranging from 40% to 80%. Recent advancements include molecular methods such as Polymerase Chain Reaction (PCR) and Loop-Mediated Isothermal Amplification (LAMP), offering improved sensitivity and specificity. Serological tests like Widal test are losing favor due to their low sensitivity and specificity.

Antimicrobial Therapy

Fluoroquinolones, the mainstay of treatment, are now compromised due to increasing resistance. Current guidelines recommend the use of third-generation cephalosporins like ceftriaxone or azithromycin as first-line therapy. The duration of therapy is typically 10-14 days.

Vaccination

Prevention through vaccination is crucial in endemic areas. Two vaccines are currently available: the Vi polysaccharide vaccine and the Ty21a vaccine. Both vaccines are moderately effective and safe but are not recommended for children under two years of age.

Resistance and Surveillance

Antimicrobial resistance is a significant concern in the management of enteric fever. Continuous surveillance of antimicrobial resistance patterns is essential to guide therapy. The World Health Organization recommends the use of Integrated Surveillance of Antimicrobial Resistance (GLASS) for this purpose.

Conclusion

Enteric fever continues to pose a significant health challenge. Advances in diagnostic methods and treatment protocols, along with vaccination and surveillance of antimicrobial resistance, are crucial in managing this disease. Healthcare professionals should stay updated on these advancements to ensure optimal patient care.
