Acute Coronary Syndrome (ACS) represents a spectrum of conditions associated with sudden, reduced blood flow to the heart. The management of ACS has witnessed significant advancements over the past decade, enhancing patient outcomes. This article provides an overview of these advancements and best practices in ACS management.
Primary percutaneous coronary intervention (PCI) has emerged as the preferred reperfusion strategy in ST-segment elevation myocardial infarction (STEMI). The advent of drug-eluting stents has significantly reduced restenosis rates. Moreover, the use of intravascular ultrasound and optical coherence tomography has improved stent placement and optimization.
Pharmacotherapy has also evolved, with the introduction of more potent antiplatelet agents such as ticagrelor and prasugrel. These medications have shown to reduce recurrent ischemic events in ACS patients.
Early diagnosis and risk stratification using tools like the GRACE or TIMI risk scores are crucial in ACS management. Rapid reperfusion therapy, either by PCI or fibrinolysis, is the cornerstone of treatment in STEMI patients. Non-STEMI patients benefit from an early invasive strategy, especially those at high risk.
Optimal medical therapy, including dual antiplatelet therapy (DAPT), statins, beta-blockers, and ACE inhibitors, is essential in all ACS patients. Careful attention to medication adherence and lifestyle modifications can significantly improve long-term outcomes.
ACS management has undergone significant advancements, particularly in the areas of revascularization techniques and pharmacotherapy. Best practices emphasize early diagnosis, risk stratification, rapid reperfusion, and optimal medical therapy. Continuing education and adherence to these principles are crucial in delivering high-quality care to ACS patients and improving their outcomes.
