Diagnosis is based on the presence of severe hypertension along with evidence of acute target organ damage. This requires a thorough history, physical examination, and appropriate diagnostic tests.
Treatment of hypertensive emergencies involves immediate reduction of BP, typically by 25% within the first hour. Intravenous antihypertensive medications are preferred for rapid action. The choice of drug depends on the affected organ system, with sodium nitroprusside, labetalol, and nicardipine being commonly used. Care must be taken to avoid overly rapid reduction of BP, which can lead to ischemic complications.
Close monitoring of patients is mandatory during the acute phase of treatment. Regular assessment of BP, renal function, cardiac status, and neurological condition is essential. Once the emergency is stabilized, the patient should be transitioned to oral antihypertensive therapy. Long-term follow-up and lifestyle modifications are key to preventing recurrences.
