A detailed history can provide valuable clues. The onset, progression, and distribution of the rash, associated symptoms, recent travel, medication history, and exposure to infectious diseases should be thoroughly explored. Special attention should be given to any associated systemic symptoms, such as arthralgia, sore throat, or cough.
A thorough physical examination, including a detailed dermatological examination, is imperative. The nature of the rash (macular, papular, vesicular, etc.), its distribution and characteristics can provide significant diagnostic clues. Laboratory investigations should be guided by history and examination findings. These may include complete blood count, liver function tests, serological tests for specific infections, and skin biopsy if indicated.
Based on the above, a differential diagnosis can be formulated. It is important to consider both common and rare causes. Common causes include viral exanthems, drug reactions, and autoimmune diseases. Rare causes include vasculitis, Kawasaki disease, and infectious diseases like measles or Lyme disease. The diagnostic approach should be iterative, and the initial diagnosis should be reassessed if the patient does not improve with treatment.
Diagnosing fever with rash requires a comprehensive, systematic approach. Early recognition of life-threatening conditions is essential. A thorough history, detailed physical and dermatological examination, and appropriate investigations are key to making an accurate diagnosis. The diagnostic approach should be iterative, considering both common and rare causes, and should be reassessed if the patient does not respond to treatment as expected.
