Non-Alcoholic Fatty Liver Disease (NAFLD) is a significant global health concern, affecting approximately 25% of the world's population. Characterised by the accumulation of fat in liver cells, NAFLD can progress to more severe conditions like Non-Alcoholic Steatohepatitis (NASH), cirrhosis, and hepatocellular carcinoma. Understanding and managing this disease is crucial to prevent its progression and associated complications.
NAFLD is a multifactorial disease, often associated with metabolic syndrome. The pathophysiology involves insulin resistance, oxidative stress, and lipid metabolism dysregulation. Genetic predisposition, dietary habits, and gut microbiota are also significant contributing factors.
Diagnosis of NAFLD often occurs incidentally during routine blood tests or imaging studies. Elevated liver enzymes, particularly alanine aminotransferase (ALT), can hint towards NAFLD. Ultrasound is the most commonly used imaging modality, but it has limitations in detecting mild steatosis. Liver biopsy, though invasive, remains the gold standard for diagnosing and staging NAFLD.
Management of NAFLD focuses on lifestyle modifications, such as diet changes and increased physical activity, aiming to reduce weight and improve insulin resistance. Pharmacological interventions are considered for patients with NASH or fibrosis. Emerging therapies, including drugs targeting metabolic pathways and fibrosis, are under investigation.
NAFLD's prognosis depends on the stage of the disease and the presence of fibrosis. Regular follow-up with liver function tests and imaging is recommended to monitor disease progression. Patients with advanced NAFLD require surveillance for hepatocellular carcinoma.
In summary, NAFLD is a prevalent disease with potential severe outcomes. Early diagnosis and effective management strategies are vital to prevent disease progression. Ongoing research aims to improve understanding of NAFLD's pathophysiology and develop novel therapeutic interventions.
