Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, poses a significant global health burden. Despite advancements in healthcare, managing this illness remains a challenge due to the increasing resistance to antibiotics. This article aims to provide a comprehensive guide to healthcare professionals for the effective management of enteric fever.
Early diagnosis is crucial for effective management. Blood culture remains the gold standard for diagnosis, although molecular methods like PCR are increasingly being used for their higher sensitivity and specificity. Rapid diagnostic tests, while less specific, can provide early presumptive diagnosis.
Antibiotic therapy forms the cornerstone of treatment. The choice of antibiotic should be guided by local susceptibility patterns. Fluoroquinolones, ceftriaxone, and azithromycin are commonly used. However, the emergence of extensively drug-resistant strains necessitates regular surveillance and judicious use of antibiotics.
Supportive care is integral to management. Adequate hydration, correction of electrolyte imbalances, and management of complications such as intestinal perforation or haemorrhage are vital. Nutritional support plays a key role in enhancing the patient's immune response and promoting recovery.
Vaccination is an effective preventive measure against enteric fever. Two vaccines, Vi polysaccharide and Ty21a, are currently available and recommended for high-risk groups and travellers to endemic areas. Efforts are ongoing to develop more effective vaccines.
Effective management of enteric fever requires a multi-pronged approach, including early diagnosis, appropriate antibiotic therapy, supportive care, and vaccination. Continuous surveillance and research are needed to address the challenges posed by antibiotic resistance and to develop more effective vaccines. Healthcare professionals play a pivotal role in implementing these strategies and mitigating the global burden of enteric fever.
