Chronic cough, defined as a cough persisting for more than eight weeks, presents a diagnostic challenge to clinicians due to its multifactorial etiology. This article outlines a comprehensive approach to diagnosing and treating this common yet perplexing condition.
Initial evaluation should focus on common causes such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). History taking and physical examination can guide further investigations, including chest radiography, spirometry, and testing for GERD. If these investigations are inconclusive, rarer causes such as bronchiectasis, lung cancer, and non-asthmatic eosinophilic bronchitis should be considered.
Treatment should be tailored to the underlying cause. In cases of GERD, proton pump inhibitors and lifestyle modifications are first-line therapies. For asthma-related cough, inhaled corticosteroids and bronchodilators are recommended. Antihistamines and decongestants can alleviate UACS symptoms. If the cough remains unresponsive, referral to a specialist may be necessary for further diagnostic testing and treatment.
A multidisciplinary approach involving pulmonologists, gastroenterologists, and otolaryngologists can provide a comprehensive evaluation and treatment plan. This collaborative approach can lead to improved patient outcomes and satisfaction.
The development of new diagnostic tools and treatments, including molecular markers and targeted therapies, may revolutionize the management of chronic cough. Further research is needed to validate these promising advances.
Chronic cough is a complex condition requiring a systematic and comprehensive approach to diagnosis and treatment. By considering common and rarer causes, tailoring treatment to the underlying etiology, and utilizing a multidisciplinary team, clinicians can effectively manage this challenging condition. Continued research into novel diagnostic and therapeutic strategies will further enhance our ability to treat chronic cough.
