Unexplained weight loss (UWL) is a common yet perplexing clinical presentation, often indicative of underlying systemic disease. Accurate diagnosis requires a comprehensive and systematic approach, considering the broad differential diagnoses associated with this symptom.
The first step is a thorough history and physical examination. It is essential to quantify the weight loss and establish a timeline. Identifying associated symptoms, such as fatigue, gastrointestinal disturbances, or changes in appetite, can provide valuable clues. A detailed social, family, and medication history may also uncover potential etiologies.
Initial laboratory tests should include a complete blood count, metabolic panel, liver function tests, thyroid function tests, HIV test, and urinalysis. Depending on the clinical suspicion, further investigations such as CT scans or endoscopies may be warranted.
In the elderly, malignancies and depression are common causes of UWL. In younger patients, gastrointestinal disorders, endocrine diseases, and infections should be considered. In all age groups, psychiatric illnesses such as depression or eating disorders can present as UWL.
If the initial evaluation is inconclusive, regular follow-ups are crucial. Monitoring for the development of new symptoms or changes in the patient's condition can guide further diagnostic steps. Referral to a specialist may be necessary if the cause remains elusive.
UWL is a complex symptom that requires a methodical and patient-centered approach. By maintaining a high index of suspicion, conducting a thorough initial assessment, and performing appropriate investigations, physicians can effectively diagnose and manage the underlying causes of unexplained weight loss.
