Introduction

Non-Alcoholic Fatty Liver Disease (NAFLD) is a growing public health concern, with an estimated global prevalence of 25%. It is a spectrum of liver conditions not associated with excessive alcohol consumption, ranging from simple steatosis to non-alcoholic steatohepatitis, which can progress to cirrhosis and hepatocellular carcinoma. Despite its prevalence, NAFLD often remains underdiagnosed and poorly managed.

Diagnosis of NAFLD

NAFLD is often identified incidentally in routine blood tests or imaging studies. It can present with elevated liver enzymes, but up to 78% of patients may have normal levels. Ultrasound is the first-line imaging modality but lacks sensitivity for mild steatosis. More accurate diagnostic tools include transient elastography and magnetic resonance elastography. Liver biopsy remains the gold standard but is invasive and not routinely performed due to its risks.

Management Strategies

Current management strategies for NAFLD focus on reducing cardiovascular risk and slowing disease progression. Lifestyle modifications, primarily weight loss and exercise, are the first line of treatment. Pharmacological therapies, such as vitamin E and pioglitazone, have shown some benefit in non-diabetic patients with advanced fibrosis. Bariatric surgery may be considered in obese patients who fail lifestyle interventions. Emerging therapies, including FXR agonists and GLP-1 receptor agonists, show promise but require further investigation.

Role of Healthcare Professionals

Healthcare professionals play a crucial role in the early detection and management of NAFLD. Regular monitoring of patients at risk, such as those with obesity and type 2 diabetes, is essential. Education about lifestyle modifications and potential complications is vital. Multidisciplinary care involving hepatologists, dieticians, and exercise physiologists may improve outcomes.

NAFLD is a prevalent and potentially serious disease that often goes unrecognized. Early diagnosis and effective management are key to preventing its progression and associated complications. As healthcare professionals, we must stay abreast of the latest developments in NAFLD diagnosis and treatment to provide optimal care to our patients.
