This article aims to provide a comprehensive approach to evaluating abdominal pain in clinical practice.
The initial assessment should involve a thorough history taking and physical examination. The nature, onset, duration, location, and radiation of pain, associated symptoms, and exacerbating or relieving factors should be meticulously noted. The physical examination should focus on inspecting, palpating, percussing, and auscultating the abdomen.
Investigations should be guided by the clinical suspicion. Basic investigations like complete blood count, liver function tests, renal function tests, and urinalysis are usually ordered. Imaging studies like ultrasound, CT scan, or MRI may be needed based on the clinical scenario. In some cases, endoscopic procedures may be warranted.
Management is directed towards the underlying cause. It may range from conservative measures like dietary modifications and medications to surgical interventions. In some cases, a multidisciplinary approach involving gastroenterologists, surgeons, radiologists, and pain management specialists may be required.
Follow-up is crucial in patients with abdominal pain. It helps to assess the response to treatment, monitor for complications, and reassess the diagnosis if the patient is not improving. It also provides an opportunity to address the patient's concerns and expectations.
Evaluating abdominal pain requires a systematic approach involving a thorough history, physical examination, appropriate investigations, and targeted management. Regular follow-up is essential to ensure optimal patient outcomes. By adopting such a comprehensive approach, clinicians can effectively decipher the enigma of abdominal pain in their daily practice.
