Tuberculosis (TB), despite being a preventable and curable disease, continues to be a significant global health issue. The key to successful TB control lies in early detection and prompt treatment. This article provides an overview of the recent advancements in TB detection methods.
The GeneXpert MTB/RIF assay, a fully automated nucleic acid amplification test, has revolutionized TB detection. It simultaneously detects Mycobacterium tuberculosis complex (MTBC) and resistance to rifampicin within two hours. This rapid, sensitive, and specific test has significantly improved the diagnostic landscape, especially in settings with high HIV prevalence.
Line Probe Assays (LPAs) are WHO-endorsed tests that detect MTBC and resistance to rifampicin and isoniazid. LPAs are faster than conventional drug susceptibility testing methods and have been instrumental in the early detection of multidrug-resistant TB (MDR-TB) cases.
Next-Generation Sequencing (NGS) is a powerful tool that provides comprehensive information about the TB genome. It can identify drug resistance mutations, track transmission patterns, and offer insights into the evolution and adaptation of the TB bacterium. Although currently limited by cost and technical requirements, NGS holds great potential for TB diagnosis and research.
Immunological tests, such as interferon-gamma release assays (IGRAs), have made strides in diagnosing latent TB infection. These tests measure the immune response to TB antigens and are more specific than the traditional tuberculin skin test.
With the advent of advanced diagnostic methods, the fight against TB has entered a new phase. These technologies offer enhanced sensitivity, specificity, and speed, enabling faster detection and treatment initiation. However, accessibility and affordability remain challenges in resource-limited settings. Continued research and investment are crucial to make these advanced diagnostic tools universally available and to ultimately eradicate TB.
