Thyroid nodules, often an incidental finding, are clinically significant due to their potential malignancy. This guide aims to provide a comprehensive overview of their evaluation, aiding healthcare professionals in their clinical decision-making process.
The prevalence of thyroid nodules is increasing with improved imaging techniques. Despite the majority being benign, the risk of malignancy underscores their clinical importance. Therefore, accurate evaluation is imperative for appropriate management and patient counseling.
The initial evaluation involves a thorough history and physical examination. Risk factors such as radiation exposure, family history, and symptoms of dysphonia, dysphagia, or rapid growth should raise suspicion for malignancy. A neck examination can provide valuable information about nodule size, consistency, and presence of cervical lymphadenopathy.
High-resolution ultrasound is the primary modality for thyroid nodule characterization. It can determine the size, composition, echogenicity, and vascularity of the nodule, which are crucial for risk stratification. Suspicious features on ultrasound include microcalcifications, irregular margins, taller-than-wide shape, and hypoechogenicity.
Based on the clinical and ultrasound risk stratification, further evaluation may include a thyroid function test and fine-needle aspiration biopsy (FNAB). FNAB is the most reliable method to differentiate benign from malignant nodules. Molecular testing can be considered when FNAB results are indeterminate.
The management of thyroid nodules depends on the risk of malignancy and patient's symptoms. Options range from surveillance for low-risk nodules to surgery for high-risk nodules. The decision should be individualized, considering patient's preference and comorbidities.
Thyroid nodule evaluation is a complex process requiring comprehensive assessment. A systematic approach involving initial evaluation, diagnostic imaging, further evaluation, and individualized management can effectively stratify the risk and guide the treatment strategy, enhancing patient care.
