Parkinson's disease (PD) is a progressive neurodegenerative disorder primarily affecting the motor system. It's characterized by four cardinal symptoms: bradykinesia, resting tremor, rigidity, and postural instability. Understanding its pathophysiology, diagnosis, and management is essential for healthcare professionals to provide optimal patient care.
PD is primarily associated with the loss of dopaminergic neurons in the substantia nigra pars compacta, leading to a reduction in striatal dopamine. This imbalance in the basal ganglia circuitry results in the characteristic motor symptoms. The presence of Lewy bodies, intracellular inclusions containing alpha-synuclein, is a pathological hallmark of PD.
Diagnosis of PD is clinical, based on history and physical examination. There are no definitive diagnostic tests. The presence of at least two of the four cardinal symptoms, with one being either resting tremor or bradykinesia, suggests PD. Neuroimaging can help exclude other causes of parkinsonism but is not routinely used in diagnosis.
Management of PD aims to improve quality of life by controlling symptoms. Pharmacological therapy, mainly dopaminergic medications, is the mainstay of treatment. Non-pharmacological interventions, including physiotherapy and occupational therapy, are also crucial. Deep brain stimulation may be considered in patients with severe motor fluctuations unresponsive to medication.
PD is a chronic, progressive disease. The rate of progression varies widely among individuals. While it is not fatal, complications can significantly affect quality of life and can be life-threatening. Regular follow-up and adjustment of management plans are crucial in maintaining optimal patient functioning and well-being.
