Stroke, a leading cause of morbidity and mortality worldwide, often presents as a medical emergency. Early recognition and prompt management are critical to improve patient outcomes. This article outlines key strategies for early stroke recognition and management.
Stroke symptoms often appear suddenly and vary depending on the affected brain area. The FAST (Face drooping, Arm weakness, Speech difficulties, and Time to call emergency services) acronym is a widely recognized tool for identifying potential stroke. However, clinicians should also consider other signs such as sudden vision changes, dizziness, and severe headache.
Upon suspecting a stroke, immediate diagnostic measures should be initiated. Neuroimaging, particularly non-contrast computed tomography (CT), is the initial diagnostic modality of choice. It helps identify the stroke type, guiding further management. Other investigations like blood tests and electrocardiogram may be necessary to rule out stroke mimics and identify potential etiologies.
Once a stroke is confirmed, prompt initiation of treatment is crucial. For ischemic strokes, intravenous thrombolysis with recombinant tissue plasminogen activator (rtPA) is the standard of care if administered within 4.5 hours of symptom onset. In select cases, endovascular thrombectomy may be considered. For hemorrhagic strokes, management is primarily supportive, focusing on blood pressure control and neurosurgical interventions when necessary.
Stroke care is multidisciplinary, involving neurologists, radiologists, nurses, and rehabilitation therapists. Coordinated care through a stroke team or stroke unit can significantly improve patient outcomes.
Early recognition and management of stroke are pivotal to minimize neurological damage and improve patient outcomes. Clinicians should familiarize themselves with the signs of stroke, employ appropriate diagnostic strategies, and initiate prompt treatment. Multidisciplinary care is integral to effective stroke management.
