Acute Coronary Syndrome (ACS) remains a significant cause of morbidity and mortality worldwide, necessitating constant evolution in its management strategies. The aim of this article is to provide an updated approach to ACS management, focusing on optimizing patient outcomes.
Early recognition of ACS, facilitated by clinical assessment and diagnostic tools such as ECG and cardiac biomarkers, is critical. Risk stratification using validated tools like the GRACE score helps identify high-risk patients who would benefit most from aggressive intervention.
Timely reperfusion remains the cornerstone of ACS management. Primary Percutaneous Coronary Intervention (PCI) is the preferred reperfusion strategy if it can be performed within 120 minutes of first medical contact. Otherwise, fibrinolytic therapy should be considered.
Antithrombotic therapy plays a pivotal role in ACS management. Dual antiplatelet therapy (DAPT) comprising aspirin and a P2Y12 inhibitor is recommended. The choice of P2Y12 inhibitor should be individualized based on the patient's bleeding risk and the planned revascularization strategy.
Secondary prevention measures, including lifestyle modification and control of risk factors, are crucial in preventing recurrent events. Statin therapy, ACE inhibitors, and beta-blockers form the cornerstone of medical therapy for secondary prevention.
Optimizing patient outcomes in ACS requires a comprehensive approach encompassing early recognition, risk stratification, timely reperfusion, antithrombotic therapy, and secondary prevention measures. As our understanding of ACS evolves, it is crucial to continually update our management strategies to reflect the latest evidence and best practices.
