Chronic cough, defined as a cough lasting more than eight weeks, is a prevalent and challenging condition in clinical practice. Despite its prevalence, the complex etiology of chronic cough often makes diagnosis and management a conundrum for clinicians. This article aims to provide a comprehensive approach to unravel this mystery.
Chronic cough can be attributed to a myriad of causes, with the most common being upper airway cough syndrome (UACS), gastroesophageal reflux disease (GERD), and asthma. It's crucial to understand that these conditions may coexist, complicating the diagnostic process. The pathophysiology of chronic cough is multifactorial, involving heightened cough reflex sensitivity and neuronal hypersensitivity.
Given the multifactorial nature of chronic cough, a systematic approach is essential. A thorough history and physical examination are the cornerstones of evaluation. Additional investigations, such as chest radiographs, spirometry, and upper GI endoscopy, may be warranted based on clinical suspicion. Empirical therapy trials can also aid in diagnosis.
Management of chronic cough should be tailored to the identified underlying cause(s). For UACS, antihistamines and nasal steroids may be beneficial. GERD-related coughs may respond to proton pump inhibitors, while inhaled corticosteroids are the mainstay for asthma-related cough. Non-pharmacological measures, including behavioral therapy, can also play a crucial role in managing refractory cases.
Chronic cough is a complex clinical entity that necessitates a comprehensive and systematic approach for effective diagnosis and management. Understanding the common etiologies and their pathophysiology, combined with a thorough clinical evaluation and targeted treatment strategies, can help clinicians unravel the mystery of chronic cough, thereby improving patient outcomes and quality of life.
