Chronic cough, defined as a cough lasting more than eight weeks, presents a common yet challenging issue in clinical practice. Despite its prevalence, the etiology often remains elusive, leading to diagnostic dilemmas and management difficulties.
Traditionally, chronic cough has been attributed to postnasal drip syndrome, gastroesophageal reflux disease, and asthma. However, recent studies have highlighted the role of atypical presentations, including eosinophilic bronchitis and non-acid reflux. Moreover, the concept of cough hypersensitivity syndrome is gaining traction, emphasizing the role of neurogenic inflammation and peripheral sensitization in the pathophysiology of chronic cough.
Given the multifactorial nature of chronic cough, a systematic and comprehensive approach is crucial. A thorough history and physical examination can provide valuable clues. Additionally, objective tests such as spirometry, chest radiography, and upper airway endoscopy can help in identifying the underlying condition. More advanced investigations like bronchoscopy, esophageal manometry, and 24-hour pH monitoring may be warranted in refractory cases.
Treatment should be tailored according to the identified etiology. Empirical therapy based on the most common causes may provide relief in many cases. However, refractory cases may require a more targeted approach, including inhaled corticosteroids, proton pump inhibitors, or neuromodulators. Recent advances include the use of novel antitussive drugs targeting neuronal pathways, providing promising results in clinical trials.
In conclusion, chronic cough requires a comprehensive approach for effective diagnosis and management. Understanding the evolving concepts in etiology and pathophysiology, coupled with a systematic diagnostic approach and targeted treatment strategies, can enhance patient outcomes and reduce the burden of this common condition. As our understanding of chronic cough continues to expand, it is hoped that more effective and tailored treatment strategies will emerge.
