The initial evaluation of GIB should focus on stabilization and assessment of the patient's hemodynamic status. A detailed history and physical examination can provide vital clues to the source and severity of the bleeding. Laboratory tests, including complete blood count, coagulation profile, and liver function tests, are essential for evaluating the bleeding risk and guiding resuscitation efforts.
The next step involves localizing the source of bleeding. Upper GIBs, originating proximal to the ligament of Treitz, are typically evaluated with esophagogastroduodenoscopy (EGD). Lower GIBs, on the other hand, may require colonoscopy or capsule endoscopy for diagnosis. Advanced imaging techniques like CT angiography can be employed when endoscopic methods fail to identify the bleeding source.
Management of GIB primarily involves hemodynamic stabilization, followed by specific treatment based on the identified source. Endoscopic interventions, such as sclerotherapy, band ligation, or argon plasma coagulation, are often the first-line treatment. More severe or refractory cases may necessitate interventional radiology procedures or surgical intervention. Importantly, the management should also include risk stratification and preventive measures for rebleeding.
In conclusion, the clinical approach to GIB involves a systematic process of initial stabilization, diagnosis, and targeted treatment.
