A chronic cough, persisting for over eight weeks in adults, or over four weeks in children, is a common yet challenging clinical presentation. The diagnosis and management of chronic cough require a comprehensive and systematic approach.
Initial evaluation should include a detailed history and physical examination to identify common causes such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). Investigations may include chest radiography, spirometry, and other tests as indicated by clinical features. An empirical therapeutic trial may sometimes be useful in confirming the diagnosis.
Refractory chronic cough, or chronic cough unresponsive to guideline-based treatment, can be particularly challenging. In these cases, rarer causes such as lung cancer, non-asthmatic eosinophilic bronchitis, and psychogenic cough should be considered. A multidisciplinary approach involving pulmonologists, gastroenterologists, and ear, nose, and throat specialists may be beneficial.
Treatment should be etiology-specific and patient-centered. For example, proton pump inhibitors for GERD, inhaled corticosteroids for asthma, or antihistamines for UACS. Non-pharmacological interventions such as speech therapy and behavioral therapy may also be effective. In refractory cases, novel therapies such as neuromodulators are showing promise.
Patients often have misconceptions about chronic cough that can impact their quality of life and adherence to treatment. Educating patients about the nature of their condition, the importance of adherence to treatment, and the realistic expectations of therapy can significantly improve outcomes.
Chronic cough is a complex condition requiring a comprehensive and systematic approach to diagnosis and management. Understanding the common and rarer causes, utilizing a multidisciplinary approach, employing etiology-specific treatments, and focusing on patient education are key to improving patient outcomes.
