Common etiologies of chronic cough include upper airway cough syndrome (UACS), gastroesophageal reflux disease (GERD), and asthma. However, the pathophysiology is multifactorial, involving mechanisms such as heightened cough reflex sensitivity, neurogenic inflammation, and eosinophilic airway inflammation.
An organized, systematic approach is crucial in diagnosing chronic cough. Initial evaluation should include a thorough history and physical examination, focusing on identifying common causes and risk factors. If the initial evaluation is unremarkable, further diagnostic testing such as chest radiography, spirometry, and bronchoprovocation testing may be required.
Treatment should be tailored based on the underlying cause. Empirical treatment trials for UACS, GERD, and asthma may be considered. If no specific cause is identified, symptomatic treatment with cough suppressants and neuromodulators may be beneficial. Refractory cases may require referral to a specialist.
Emerging therapies targeting the neural pathways of cough, such as P2X3 receptor antagonists, have shown promising results in recent clinical trials. These novel therapies may play a significant role in the management of refractory chronic cough in the future.
In conclusion, chronic cough is a complex clinical entity that requires a comprehensive, systematic approach for effective diagnosis and management. Understanding the etiology, pathophysiology, and treatment options is vital for optimal patient care. The advent of novel therapies offers hope for those with refractory chronic cough, signifying a promising future in its management.
