Chronic cough, defined as a cough lasting more than eight weeks, is a prevalent symptom in clinical practice. Despite its frequency, it often presents a diagnostic challenge to healthcare providers due to its multifaceted etiology.
An effective diagnostic approach is essential in managing chronic cough. A thorough patient history and physical examination can provide valuable clues. The most common causes include upper airway cough syndrome (UACS), gastroesophageal reflux disease (GERD), and asthma. If these conditions are ruled out, less common causes such as non-asthmatic eosinophilic bronchitis, lung cancer, and chronic obstructive pulmonary disease (COPD) should be considered.
Diagnostic tests should be tailored to the suspected underlying condition. Chest radiographs and spirometry are often the first-line investigations. When these tests are non-revealing, high-resolution computed tomography (HRCT), bronchoscopy, and esophageal pH monitoring may be necessary. Empirical therapy trials can also provide diagnostic and therapeutic information, especially when the cause is unclear.
Management should be focused on treating the underlying cause. For UACS, antihistamines and nasal corticosteroids are often effective. GERD-related cough can be managed with proton pump inhibitors, while inhaled corticosteroids are the mainstay of treatment for asthma-related cough. If the cough is refractory to treatment, referral to a specialist may be necessary.
Chronic cough is a common yet complex clinical problem. A systematic approach to diagnosis, incorporating a comprehensive history, physical examination, and appropriate diagnostic tests, can help identify the underlying cause. Tailored management strategies can then be implemented, improving patient outcomes and reducing the burden of this often debilitating symptom.
