Peptic ulcer disease (PUD) is a common gastrointestinal disorder characterized by the occurrence of ulcers in the stomach and duodenum. With the advent of new diagnostic tools and therapeutic strategies, the management of PUD has significantly evolved over the past decade.
Diagnosis of PUD has been revolutionized with the advent of endoscopic techniques, which provide direct visualization of the gastrointestinal tract. Additionally, non-invasive tests for Helicobacter pylori, the primary causative agent of PUD, have improved early detection and treatment initiation.
Pharmacological management of PUD has seen substantial advancements. Proton pump inhibitors (PPIs) remain the cornerstone of treatment, with newer generation PPIs offering improved acid suppression. The development of quadruple therapy, combining a PPI, bismuth, and two antibiotics, has significantly increased H. pylori eradication rates.
For PUD complications such as bleeding, endoscopic therapies have emerged as first-line treatment. Techniques such as endoscopic clipping and band ligation have shown high efficacy in controlling ulcer bleeding. Moreover, endoscopic submucosal dissection (ESD) has provided a less invasive alternative to surgery for gastric ulcers.
While the role of surgery in PUD has diminished with the advent of effective medical and endoscopic treatments, it remains crucial for managing complications like perforation and gastric outlet obstruction. Laparoscopic techniques have minimized surgical morbidity and improved patient outcomes.
Advancements in the management of PUD have led to improved patient outcomes and reduced disease burden. These include novel diagnostic approaches, pharmacological innovations, the emergence of endoscopic therapies, and the refinement of surgical techniques. As we continue to understand the pathophysiology of PUD, further advancements in its management are anticipated.
