Traditional diagnostic methods, such as blood culture and Widal test, have limitations in terms of sensitivity, specificity, and turnaround time. Recent advancements have led to the development of molecular diagnostic tests like Polymerase Chain Reaction (PCR) and Nucleic Acid Amplification Tests (NAATs), which offer improved sensitivity and specificity, and faster results.
Increasing antimicrobial resistance, particularly to fluoroquinolones and third-generation cephalosporins, has complicated the management of enteric fever. This necessitates the use of culture and sensitivity tests to guide antibiotic therapy. Empiric treatment should now consider the local resistance patterns.
Vaccination plays a pivotal role in the management of enteric fever. The World Health Organization recommends the use of Ty21a and Vi polysaccharide vaccines in endemic areas. The newer conjugate vaccines, which have a longer duration of protection and can be given to younger children, are promising additions to the prevention strategy.
Supportive care, including hydration and antipyretics, remains a mainstay of treatment. In severe or complicated cases, hospitalization may be required. Post-treatment follow-up is crucial to ensure complete resolution of the disease and to monitor for potential complications.
An updated approach to the diagnosis and management of enteric fever includes the use of advanced diagnostic techniques, consideration of local antimicrobial resistance patterns, vaccination, and comprehensive patient care. As the disease landscape changes, it is essential for clinicians to stay informed about the latest developments to provide the best care for their patients.
