Acute chest pain represents one of the most challenging diagnostic dilemmas in clinical practice. With a wide array of potential etiologies, ranging from benign to life-threatening, a systematic approach is essential to ensure efficient and effective patient care.
Upon presentation, a rapid evaluation of vital signs and patient history should be completed to identify immediate threats to life. The clinician should consider risk factors such as age, gender, smoking, family history, and presence of comorbidities like hypertension and diabetes.
Electrocardiogram (ECG) is the first-line diagnostic tool in chest pain evaluation. However, it is crucial to remember that a normal ECG does not exclude significant cardiac pathology. Additional investigations such as troponin levels, chest X-ray, or computed tomography angiography may be necessary based on the clinical suspicion.
Management strategies should be individualized, based on the presumed etiology of chest pain. For suspected acute coronary syndrome, immediate reperfusion therapy is indicated. For other conditions such as pulmonary embolism or aortic dissection, specific treatments are required.
Patients with chest pain should be risk-stratified to guide follow-up. Those with high-risk features may require further diagnostic testing, while low-risk patients may be safely discharged with appropriate follow-up.
Deciphering the cause of acute chest pain requires a comprehensive, systematic approach, involving initial assessment, diagnostic investigations, management, and follow-up. By maintaining a high index of suspicion and utilizing a patient-centered approach, clinicians can ensure optimal outcomes for patients presenting with this common but complex symptom.
