Chronic cough, defined as a persistent cough lasting over eight weeks, poses a significant diagnostic challenge to clinicians. It often results in substantial patient distress and impaired quality of life. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough in clinical practice.
Diagnosis of chronic cough involves a thorough history taking and physical examination to identify potential causes. An understanding of the cough reflex and the concept of cough hypersensitivity syndrome is crucial. The patient's exposure history, associated symptoms, and response to previous treatments provide valuable clues. Investigations including chest radiography, spirometry, and when indicated, bronchoscopy or CT scan should be considered.
Management of chronic cough should be tailored according to the underlying cause. Empiric treatment trials targeting the most common causes - gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS), and asthma - can be considered in the absence of specific diagnostic clues. Non-pharmacological approaches including speech therapy and behavioral modification have shown promising results.
Emerging therapies targeting neural pathways involved in cough reflex are under investigation. Drugs like P2X3 receptor antagonists have shown potential in reducing cough frequency in patients with refractory chronic cough. However, more extensive studies are needed to establish their safety and efficacy.
Chronic cough represents a complex interplay of various factors leading to cough hypersensitivity. A structured, patient-centered approach is required for effective management. While empiric treatments remain the mainstay, emerging therapies offer hope for patients with refractory chronic cough. Continuous research and clinical trials are essential to further our understanding and management of this challenging condition.
