Chronic cough, defined as a cough persisting for more than eight weeks, is a common yet challenging condition encountered in primary care. It significantly impacts the quality of life and necessitates a comprehensive understanding for effective management.
The pathophysiology of chronic cough is multifactorial, often associated with conditions such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). However, in some cases, the cause remains unidentifiable, leading to the term 'idiopathic chronic cough'.
A thorough history and physical examination form the cornerstone of diagnosis. Investigations such as chest X-ray, spirometry, and bronchoscopy may be warranted based on clinical suspicion. Empirical treatment trials for common causes like GERD and UACS may also aid in identifying the underlying etiology.
Treatment is primarily aimed at addressing the underlying cause. For instance, proton pump inhibitors for GERD, inhaled corticosteroids for asthma, and antihistamines for UACS. Moreover, cough suppressants and behavioural therapy play a crucial role in managing idiopathic chronic cough.
Recent advancements have introduced novel therapies targeting neuronal pathways involved in cough reflex. P2X3 antagonists, such as gefapixant, have shown promising results in reducing cough frequency, thus offering potential new avenues for treatment.
Chronic cough is a complex condition requiring a comprehensive approach for diagnosis and management. Understanding the underlying mechanisms, adopting a systematic diagnostic approach, and being aware of the latest advancements in treatment strategies are crucial for healthcare professionals in providing optimal patient care. Ongoing research into the pathophysiology of chronic cough and the development of novel treatments is likely to further improve patient outcomes in the future.
