Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, is a systemic disease that continues to pose significant health challenges in many parts of the world. This article aims to provide an updated approach to the diagnosis and management of enteric fever.
Diagnosis of enteric fever has traditionally relied on blood, bone marrow, or stool cultures. However, these methods are time-consuming and often yield false negatives. Recent advancements have introduced rapid diagnostic tests (RDTs) like Typhidot and Tubex TF, which detect IgM and IgG antibodies. Although they offer quick results, their sensitivity and specificity vary. Molecular techniques such as Polymerase Chain Reaction (PCR) and Next-Generation Sequencing (NGS) have also shown promise, providing high sensitivity and specificity.
The mainstay of treatment for enteric fever is antibiotic therapy. The choice of antibiotic is guided by local antimicrobial susceptibility patterns. Fluoroquinolones, third-generation cephalosporins, and azithromycin are currently the drugs of choice. However, the emergence of extensively drug-resistant Salmonella strains necessitates the prudent use of antibiotics. Adjunctive therapy with corticosteroids may be beneficial in severe cases.
Vaccination forms an integral part of enteric fever prevention. The WHO recommends the use of Ty21a and Vi polysaccharide vaccines in endemic areas. Recently, a new conjugate vaccine (TCV) has been prequalified by WHO, showing high efficacy and longer duration of protection. Additionally, safe water and food practices, improved sanitation, and public health surveillance are crucial for prevention.
In conclusion, the diagnosis and treatment of enteric fever require a multifaceted approach. While rapid diagnostic tests and molecular techniques improve diagnostic accuracy, the judicious use of antibiotics is key in treatment. Vaccination, along with improved sanitation practices, plays a vital role in prevention. Staying abreast of these updated strategies is essential for healthcare professionals in managing this significant global health issue.
