Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, continues to be a significant global health concern. This article offers a comprehensive overview of the recent advancements in the diagnosis and treatment strategies of this disease.
Traditional diagnostic methods such as blood cultures have limitations in sensitivity and specificity. Recent advancements include the introduction of molecular techniques like Polymerase Chain Reaction (PCR) and Next-Generation Sequencing (NGS). These techniques offer improved diagnostic accuracy, faster results, and the ability to identify antimicrobial resistance markers.
Advancements in serological testing, including the Typhidot test and Tubex TF test, offer quicker and more accurate results compared to the Widal test. These tests detect IgM and IgG antibodies against the outer membrane proteins of Salmonella, providing a more specific diagnosis.
The rise of multidrug-resistant strains of Salmonella has necessitated the development of new treatment strategies. The use of third-generation cephalosporins and fluoroquinolones has shown promising results. Moreover, the advent of azithromycin as a single-dose oral therapy offers a significant advantage in terms of cost-effectiveness and patient compliance.
There have been significant strides in vaccine development, with the Vi conjugate vaccine (Typbar-TCV®) demonstrating high efficacy and safety in endemic regions. It offers long-lasting immunity and is suitable for children under two years, a group previously underserved by available vaccines.
While challenges persist in the management of enteric fever, advancements in diagnostic techniques and treatment strategies, coupled with promising vaccine developments, provide hope for better disease control. Continuous research and development are essential to tackle the evolving challenges posed by this disease.
