Gastroesophageal Reflux Disease (GERD) is a common yet complex condition, affecting nearly 20% of the Western population. Its diagnosis and treatment require a comprehensive understanding of its pathophysiology and the latest therapeutic approaches.
GERD diagnosis is primarily clinical, relying heavily on patient-reported symptoms. However, objective tests like endoscopy, esophageal manometry, and ambulatory pH monitoring can provide valuable insight. Endoscopy is useful in assessing mucosal damage and excluding other disorders. Esophageal manometry evaluates esophageal motility, while ambulatory pH monitoring measures acid exposure in the esophagus, confirming the diagnosis in equivocal cases.
Proton pump inhibitors (PPIs) are the mainstay of GERD treatment, providing symptom relief and healing esophagitis. However, a subset of patients may not respond adequately to PPIs, necessitating the exploration of alternative therapeutic strategies.
For patients with refractory GERD, surgical interventions such as laparoscopic Nissen fundoplication can provide long-term symptom relief. Recent advancements also include minimally invasive endoscopic techniques like Stretta and Transoral Incisionless Fundoplication (TIF). These procedures aim to augment the lower esophageal sphincter, reducing gastroesophageal reflux.
Emerging therapies for GERD include novel pharmacological agents targeting different pathophysiological mechanisms, such as reflux inhibitors and esophageal mucosal protectants. Additionally, the role of dietary interventions and gut microbiota modulation in GERD management is increasingly recognized and warrants further research.
GERD is a multifaceted disorder with a broad spectrum of clinical presentations. Its effective management requires a comprehensive approach, incorporating accurate diagnosis, optimal use of pharmacotherapy, and consideration of advanced treatment options in refractory cases. As our understanding of GERD pathophysiology expands, it paves the way for novel therapeutic strategies, promising improved patient outcomes.
