Antibiotics, since their discovery, have revolutionized healthcare. However, their misuse has led to an increase in antibiotic resistance, threatening patient safety and global health. This necessitates a rational approach to antibiotic use in clinical practice.
Antibiotic stewardship programs (ASPs) aim to optimize antibiotic use, minimizing resistance while improving patient outcomes. They involve coordinated interventions designed to improve and measure the appropriate use of antibiotics by promoting the selection of the optimal antibiotic regimen.
Strategies include de-escalation of therapy, dose optimization, and therapeutic drug monitoring. De-escalation involves switching from broad-spectrum antibiotics to narrower ones once the pathogen is identified. Dose optimization ensures the right dose is administered based on individual patient characteristics. Therapeutic drug monitoring allows for dose adjustments to maximize efficacy and minimize toxicity.
Education about antibiotic resistance and the principles of antibiotic stewardship is crucial for all healthcare professionals. Collaboration between physicians, pharmacists, and infection control professionals is key to implementing effective ASPs.
Rational use of antibiotics is a collective responsibility that requires understanding, collaboration, and commitment. By integrating antibiotic stewardship principles into clinical practice, we can promote patient safety, improve treatment outcomes, and combat the global threat of antibiotic resistance.
