Unexplained weight loss (UWL), defined as an unintentional loss of more than 5% of body weight within a six-month period, poses a diagnostic challenge in clinical practice. This article aims to provide a comprehensive approach for evaluating UWL, integrating the latest research and clinical guidelines.
Initial evaluation should focus on detailed history taking and physical examination. It is crucial to determine the duration, magnitude, and any accompanying symptoms of weight loss. A comprehensive review of medications, lifestyle factors, and psychiatric evaluation may provide valuable clues.
The diagnostic approach to UWL should be systematic and guided by the findings of the initial evaluation. Tests should include basic laboratory investigations, imaging studies, and targeted evaluations based on symptoms. If initial investigations are inconclusive, a watchful waiting approach may be adopted, considering the high likelihood of a diagnosis emerging over time.
Common causes of UWL include malignancy, gastrointestinal disorders, endocrine disorders, chronic infections, and psychiatric conditions. Malignancy is the most common serious cause, found in about 30% of patients with UWL. The likelihood of malignancy increases with age, smoking history, and the presence of alarming symptoms such as anorexia and abdominal pain.
Imaging plays a pivotal role in the diagnostic workup of UWL. Computed tomography (CT) of the chest, abdomen, and pelvis is often the imaging modality of choice. However, the decision to perform imaging should be individualized, considering the patient's age, symptoms, and risk factors.
Evaluating UWL is a complex process that requires a thorough and systematic approach. Understanding the common causes, adopting a patient-centered diagnostic strategy, and utilizing appropriate investigations are key to unraveling this enigma. Ongoing research is needed to refine our approach and improve patient outcomes in this challenging clinical scenario.
