Chronic cough, defined as a cough lasting more than eight weeks, is a common complaint in the clinical practice, often leading to significant morbidity and reduced quality of life. Despite its prevalence, diagnosis and management often pose a challenge due to its multifactorial etiology.
Chronic cough can be attributed to several conditions, most commonly asthma, gastroesophageal reflux disease (GERD), and upper airway cough syndrome (UACS). Other less common causes include chronic bronchitis, bronchiectasis, and the use of angiotensin-converting enzyme inhibitors. The pathophysiology involves a hypersensitive cough reflex in response to these conditions.
A thorough patient history and physical examination are paramount in identifying the cause of chronic cough. Empirical treatment trials for the most common causes may also be beneficial. If the cough persists despite this, further diagnostic testing such as chest radiography, spirometry, and bronchoscopy may be indicated. The use of cough-specific quality of life questionnaires can also aid in assessing the severity and impact of the cough on the patient's life.
Management of chronic cough should be targeted towards the underlying cause. This may involve the use of inhaled corticosteroids for asthma, proton pump inhibitors for GERD, or antihistamines and decongestants for UACS. In cases where no specific cause can be identified, a trial of neuromodulatory drugs such as gabapentin or amitriptyline can be considered. Non-pharmacological interventions, including speech and physical therapy, may also prove beneficial.
Chronic cough is a complex condition requiring a comprehensive approach for effective diagnosis and management. Understanding its etiology and pathophysiology, coupled with a systematic diagnostic approach and targeted management strategies, can lead to improved patient outcomes and quality of life.
