Unexplained weight loss (UWL) is a symptom that often perplexes clinicians due to its broad differential diagnosis. It is essential to have a systematic approach to its evaluation, as it may signal a serious underlying condition.
UWL is defined as a loss of more than 5% of body weight over a period of 6-12 months without trying. The causes can be broadly classified into three categories: malignancy, non-malignant gastrointestinal diseases, and psychiatric disorders. However, in about 15-25% of cases, the cause remains elusive even after thorough evaluation.
The initial assessment should include a detailed history and physical examination to identify any clues that might point towards a particular diagnosis. It is also important to assess the patient's nutritional status, as malnutrition can be both a cause and a consequence of UWL.
If the initial assessment is inconclusive, further investigations should be guided by the patient's age, risk factors, and associated symptoms. Basic investigations include complete blood count, metabolic panel, thyroid function tests, and chest X-ray. More specific tests like CT scan, endoscopy, or biopsy may be warranted based on the clinical suspicion.
The management of UWL is primarily directed towards the underlying cause. In cases where no cause is identified, the focus should be on nutritional support and monitoring for the development of new symptoms or signs that might suggest a diagnosis.
Unexplained weight loss can be a diagnostic challenge in clinical practice. A comprehensive and systematic approach is crucial to identify the underlying cause and guide management. Further research is needed to improve our understanding and approach to this common but complex clinical problem.
