Anemia, a reduction in the number or hemoglobin content of red blood cells, is a common hematological disorder with diverse etiologies. It poses a diagnostic challenge due to its multifactorial nature, necessitating a comprehensive approach for accurate diagnosis.
The first step in the diagnostic approach involves a thorough history and physical examination. Key aspects include dietary habits, family history, signs of blood loss, and symptoms suggestive of systemic diseases. Laboratory investigations start with a complete blood count (CBC) and peripheral smear examination. Reticulocyte count and indices provide clues about the bone marrow response.
Anemia is classified as microcytic, normocytic, or macrocytic based on mean corpuscular volume (MCV). Microcytic anemia is often due to iron deficiency or thalassemia. Normocytic anemia may be due to acute blood loss or chronic disease, while macrocytic anemia is commonly due to vitamin B12 or folate deficiency.
Further investigations are guided by the initial findings. Iron studies, hemoglobin electrophoresis, or genetic testing may be necessary for microcytic anemia. For normocytic anemia, bone marrow examination may be required in cases of unexplained anemia. In macrocytic anemia, serum B12, folate levels, and a test for intrinsic factor antibodies may be indicated.
In patients with co-existing disorders like renal disease or malignancy, the diagnostic approach may need modification. For instance, in chronic kidney disease, erythropoietin levels may be helpful. In suspected malignancy, bone marrow examination is often warranted.
Unraveling the complexity of anemia requires a systematic approach, starting from a detailed history and physical examination, followed by basic investigations and then specific tests based on initial findings. Understanding the pathophysiological basis and being aware of the special situations can greatly aid in the diagnostic process, ensuring timely and appropriate management.
