Transitioning adolescents and young adults with chronic health conditions from pediatric to adult healthcare is a critical process, directly impacting patient outcomes, adherence, and long-term health. Smooth transition models are designed to ensure continuity, minimize care gaps, and foster patient autonomy. This review synthesizes recent evidence, elucidates mechanisms underpinning successful transitions, and offers practical recommendations for clinicians, emphasizing the importance of structured, patient-centered approaches in line with contemporary guidelines.
The transition from pediatric to adult healthcare is a pivotal period for adolescents with chronic diseases or disabilities. The process requires comprehensive planning, communication, and collaboration to maintain health outcomes and prevent deterioration. As survival rates for pediatric-onset chronic conditions improve, an increasing number of youths require ongoing adult care. Effective transition models are therefore essential in modern healthcare systems, ensuring that young people receive age-appropriate, continuous, and high-quality care as they move into adulthood.
Globally, approximately 10–15% of adolescents live with at least one chronic health condition, such as type 1 diabetes, congenital heart disease, cystic fibrosis, or juvenile idiopathic arthritis. Advances in pediatric care have led to increased survival, resulting in a growing population of young adults with complex medical needs. However, studies indicate that up to 60% of these individuals experience disruptions in care during transition, leading to increased emergency visits, hospitalizations, and suboptimal disease control. The burden extends beyond medical complications, affecting psychosocial wellbeing, educational attainment, and long-term socioeconomic outcomes.
While transition itself is not a pathophysiological process, the consequences of poorly managed transitions can result in disease exacerbation and complications. For example, gaps in care for adolescents with type 1 diabetes may precipitate poor glycemic control, increasing the risk of diabetic ketoacidosis and microvascular complications. Similarly, patients with congenital heart disease are at risk for arrhythmias or heart failure if specialized follow-up lapses. Adolescents also face unique neurodevelopmental challenges, including evolving autonomy, executive function, and risk-taking behaviors, all of which can impact self-management and health outcomes.
Key risk factors for unsuccessful transition include lack of structured transition protocols, insufficient patient education, poor communication between pediatric and adult providers, low health literacy, and socioeconomic barriers. Adolescents with intellectual disabilities, mental health comorbidities, or limited family support are particularly vulnerable. System-level factors such as fragmented care networks and inadequate insurance coverage further compound these risks, leading to increased rates of nonadherence, care disengagement, and negative health outcomes.
Clinicians may observe a range of clinical features associated with transition challenges, including missed appointments, decreased adherence to therapy, and regression in self-management skills. Psychosocial manifestations such as anxiety, depression, or social withdrawal are also common. From a systems perspective, increased emergency department utilization and preventable hospitalizations often signal transition failures. Early identification of these features can prompt timely interventions and adjustments to transition planning.
There is no singular diagnostic test for transition readiness; rather, assessment relies on validated tools such as the Transition Readiness Assessment Questionnaire (TRAQ) and the Self-Management and Transition to Adulthood with Rx = Treatment (STARx) Questionnaire. These instruments evaluate domains including self-advocacy, knowledge of ones condition, medication management, and appointment scheduling. Comprehensive assessment should begin in early adolescence and be updated regularly to inform individualized transition planning.
Effective transition management is multifaceted, involving patient education, empowerment, and close collaboration between pediatric and adult care teams. Core components include early initiation of transition discussions (ideally by age 12–14), development of individualized transition plans, and structured handover meetings. Transition coordinators or case managers can facilitate communication and navigation across services. Family engagement is crucial, but should be balanced with promoting adolescent autonomy. Ongoing monitoring and flexible support tailored to developmental stage and health literacy are essential for sustained success.
Recent advances emphasize digital health solutions, such as mobile apps for self-management, telemedicine consultations, and electronic health records with transition modules. Peer mentoring programs and youth advisory councils have demonstrated improvements in self-efficacy and engagement. Emerging models also incorporate transition clinics, where pediatric and adult providers jointly assess and plan care, minimizing discontinuity. Quality improvement initiatives guided by transition outcome measures are increasingly adopted to benchmark and refine processes. Research into biologic and gene therapies for pediatric-onset diseases also underscores the need for robust, adaptable transition pathways as treatment paradigms evolve.
Major organizations, including the American Academy of Pediatrics, American College of Physicians, and Society for Adolescent Health and Medicine, advocate for structured transition protocols. Recommendations include: early and ongoing assessment of transition readiness, designation of a transition coordinator, written transition policies, and direct communication between sending and receiving providers. Guidelines stress the importance of addressing psychosocial, educational, and vocational needs alongside medical management. Documentation of transition plans in the medical record and clear delineation of provider responsibilities are also advised.
Transitioning from pediatric to adult healthcare is a complex but essential component of lifelong disease management for youths with chronic conditions. Smooth transition models anchored in patient-centered, multidisciplinary approaches reduce care gaps, improve outcomes, and empower young adults. Advances in digital health, coordinated clinics, and evidence-based protocols offer promising avenues for optimizing transitions. Continued research, provider education, and system-level investment are necessary to ensure that all adolescents experience seamless, supportive transitions into adult-oriented care.
