Abstract

Long-term cancer survivors represent an expanding population with unique healthcare challenges, including an increased risk of functional decline and social vulnerability due to the long-lasting effects of cancer and its treatment. This review synthesizes current evidence and clinical guidelines surrounding the identification and management of functional and social vulnerabilities in this cohort. Emphasis is placed on epidemiology, mechanistic insights, risk stratification, clinical assessment tools, and evidence-based approaches to screening and intervention. Recent advances and emerging therapies are discussed, along with practical guidelines for optimizing survivorship care and improving long-term outcomes.

Introduction

The growing number of cancer survivors is a testament to advances in oncology, yet it also brings forth a new set of challenges for clinicians. Survivorship is often accompanied by persistent or late-onset sequelae that affect physical function, psychological well-being, and social integration. Functional and social vulnerabilities are key determinants of quality of life and morbidity in survivors, but these issues often remain under-recognized in routine follow-up. Comprehensive screening and timely interventions are crucial for early identification and management to mitigate adverse outcomes and optimize recovery in this population.

Key risk factors for functional vulnerability include advanced age at diagnosis, pre-existing comorbidities, high-intensity or multimodal cancer therapies, and low baseline physical activity. Socioeconomic deprivation, lack of social support, and minority status elevate the risk of social vulnerability. Other contributory factors include cognitive impairment, depression, anxiety, and polypharmacy. Stratification models integrating these risk factors facilitate targeted screening and early intervention in high-risk survivors.

Clinical Features

Functional vulnerability manifests as reduced endurance, sarcopenia, frailty, impaired balance, and difficulty with ADLs or instrumental activities of daily living (IADLs). Symptoms such as chronic pain, dyspnea, and cognitive complaints may coexist, further limiting independence. Social vulnerability is characterized by isolation, loss of social roles, financial insecurity, and reduced access to healthcare or community resources. Clinicians should maintain a high index of suspicion for subtle signs, such as unintentional weight loss, frequent falls, or withdrawal from social activities, particularly in older or socioeconomically disadvantaged survivors.

Diagnosis

Screening for functional vulnerability relies on validated tools such as the Short Physical Performance Battery (SPPB), gait speed assessment, the Clinical Frailty Scale, and patient-reported outcome measures. Social vulnerability can be assessed using instruments like the Medical Outcomes Study Social Support Survey, the Duke Social Support Index, and structured interviews addressing employment, housing, and financial stability. Comprehensive geriatric assessment (CGA) offers a multidimensional framework for evaluating both domains. Early and periodic screening is recommended, ideally integrated into routine survivorship care visits.

Treatment & Management

Management of functional vulnerability involves multidisciplinary interventions, including physical rehabilitation, occupational therapy, nutritional support, and management of comorbidities. Exercise-based programs, especially those tailored for cancer survivors, have demonstrated efficacy in improving strength, balance, and quality of life. Social vulnerability requires coordinated care with social workers, financial counselors, and community organizations to address practical needs and foster social reintegration. Cognitive-behavioral therapy and peer support groups may further mitigate psychological distress. Individualized survivorship care plans that incorporate both physical and social domains are essential for holistic recovery.

Recent Advances / Emerging Therapies

Emerging strategies in survivorship care include telehealth-based rehabilitation, digital self-management platforms, and remote monitoring of functional status using wearable devices. Novel biomarkers, such as inflammatory mediators and epigenetic age, are being explored to refine risk prediction. Integrative models that combine oncologic, geriatric, and social care pathways show promise in improving outcomes. Additionally, interventions targeting social determinants of health are gaining traction, with pilot programs demonstrating reductions in healthcare utilization and improved patient-reported outcomes.

Guideline Recommendations

Professional societies, including the American Society of Clinical Oncology (ASCO) and the National Comprehensive Cancer Network (NCCN), endorse routine assessment of functional and social needs in all long-term cancer survivors. Guidelines recommend annual or more frequent screening using validated tools, especially in older adults and high-risk groups. Interventions should be evidence-based, multidisciplinary, and tailored to individual risk profiles. Survivorship care plans must be dynamic, incorporating ongoing assessment and referral pathways to specialized services as needed.
