The key to differentiating vertigo from other forms of dizziness lies in detailed history taking and careful physical examination.
A thorough assessment of a patient with vertigo should include a complete history of the presenting symptoms, focusing on the onset, duration, frequency, and associated symptoms. Physical examination should assess the vestibular, auditory, and neurological systems. Special tests like the Dix-Hallpike maneuver and head impulse test can help diagnose benign paroxysmal positional vertigo and vestibular neuritis respectively.
Vertigo can be peripheral or central in origin. Peripheral vertigo, the most common type, is usually associated with benign conditions like benign paroxysmal positional vertigo, Meniere's disease, and vestibular neuritis. Central vertigo, though less common, may indicate serious conditions like stroke or multiple sclerosis. Central vertigo requires prompt identification and management of the underlying condition. Patient education and reassurance also play a crucial role in managing this distressing symptom.
